The first group consisted of patients who underwent radical treatment FI (16 people), the second — those who did not treat FI (16 people), and the third group consisted of patients in whom FI was not detected (20 people).</p> <p>Cytokine concentrations were measured in the progressive stage of the disease in 52 patients with psoriasis and 20 individuals in the control group (healthy individuals).The spectrum of detected cytokines included: IL-1â, IL-2, IL-4, IL-6, IL-8, IL-10, IL-17A, as well as INF-á and INF-ã. For the detection of all cytokines, except for IL-17A, reagents from Vector-Best, Russia were used. The Human ELISA MAX ™ Deluxe Set (BioLegend, USA) was used to determine the concentration of IL-17A.</p> <p>During the main course of treatment (0, 2 and 4 weeks), as well as the follow-up of patients (12, 24 and 52 weeks), the severity of psoriasis was assessed with the calculation of the PASI index. At the end of the study (week 52), the number of relapses and the total duration of remission were recorded.</p> <p><bold>Results.</bold> The level of IL-8 was increased in 90,6% of psoriasis patients with FI. In this case, significant differences were obtained in comparison with the group of patients with psoriasis without diagnosed FI and with the control group, in which an increase in the concentration of IL-8 was noted, respectively, in 65% and 30% of cases. A moderate positive correlation was found between the IL-8 level and the PASI index (<italic>r</italic><italic><sub>s</sub></italic> = 0,48; <italic>p</italic> = 3,6 × 10<sup>–4</sup>). The levels of INF-á and IL-6 in psoriasis patients with FI were increased, respectively, in 15,6% and 21,9% of cases and significantly differed from the group of practically healthy individuals (<italic>p</italic> &lt; 0,05), differences from the group of patients with psoriasis without FI was not identified. The concentration of TNF-á did not differ in all three groups. The level of IL-17A was significantly increased in comparison with practically healthy individuals both in the group of psoriasis patients with FI and in the group of patients without FI (Me = 3,3 and 4,3 pg/ml versus 0,2 pg/ml). The concentrations of five interleukin cytokines, namely IL-1â, IL-2, IL-4, IL-10 and INF-ã, did not exceed the upper limit of the reference range in all patients of the experimental (patients with psoriasis) and control groups.</p> <p>Evaluation of the PASI index during the observation of patients in the first and second groups showed significantly lower values of this indicator for weeks 24 and 52 in the first group (0,2 vs. 7,0; <italic>p</italic> = 0,02 and 0,1 vs. 7,2; <italic>p</italic> = 0,002). The relative risk of disease recurrence in the absence of radical treatment for FI was 2,6 (CI 1,1 to 5,2).</p> <p><bold>Conclusion.</bold> Data were obtained showing the role of IL-8 and focal infection, with the presence of which its increased production is associated, in the worsening of the course of psoriasis with the frequent development of relapses. Important preventive measures leading to a decrease in the number of relapses of psoriasis are timely diagnosis and radical treatment of focal infection.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> Очаги хронической инфекции (ОХИ) являются важными триггерными факторами развития псориаза, а значит, могут утяжелять течение дерматоза, в том числе и за счет повышения в крови определенных провоспалительных цитокинов.</p> <p><bold>Цель исследования.</bold> Изучить цитокиновый профиль больных псориазом в стадии обострения заболевания, а также оценить влияние радикального лечения очагов хронической инфекции на течение дерматоза.</p> <p><bold>Материал и методы.</bold> В проспективное сравнительное нерандомизированное исследование включены 52 больных вульгарным псориазом в возрасте от 18 до 65 лет, подписавших добровольное информированное согласие. В первую группу вошли пациенты, санировавшие выявленные ОХИ (16 человек), во вторую — не санировавшие (16 человек), и третью группу составили пациенты, у которых ОХИ выявлены не были (20 человек).</p> <p>Концентрации цитокинов были измерены в прогрессирующую стадию заболевания у 52 больных псориазом и 20 лиц контрольной группы (здоровые лица). Спектр определяемых цитокинов включал: IL-1â, IL-2, IL-4, IL-6, IL-8, IL-10, IL-17А, а также INF-á и INF-ã. При детекции всех цитокинов, кроме IL-17A, использовались реагенты кoмпании «Вектор-Бест», Россия. Для определения концентрации IL-17А был выбран набор Human ELISA MAX™ DeluxeSet фирмы BioLegend, США.</p> <p>В период проведения основного курса лечения (0, 2 и 4-я недели), а также последующего наблюдения пациентов (12, 24 и 52-я недели) проводилась оценка тяжести псориаза с расчетом индекса PASI. В момент завершения исследования (неделя 52) фиксировались количество рецидивов и общая продолжительность ремиссии.</p> <p><bold>Результаты.</bold> Уровень IL-8 оказался увеличенным у 90,6% больных псориазом с наличием ОХИ. В данном случае получены значимые различия в сравнении с группой больных псориазом без диагностированных ОХИ и с контрольной группой, в которых повышение концентрации IL-8 отмечалось, соответственно, в 65 и 30% случаев. Выявлена умеренная положительная корреляция между уровнем IL-8 и индексом PASI (<italic>r</italic><italic><sub>s</sub></italic> = 0,48; <italic>p</italic> = 3,6 × 10<sup>–4</sup>). Уровни INF-á и IL-6 у больных псориазом с наличием ОХИ были повышены, соответственно, в 15,6 и 21,9% случаев и значимо отличались от группы практически здоровых лиц (<italic>p</italic> &lt; 0,05), различий с группой больных псориазом без ОХИ выявлено не было. Концентрация TNF-á не отличалась во всех трех группах. Уровень IL-17А был значимо повышен в сравнении с практически здоровыми лицами как в группе больных псориазом, имеющих ОХИ, так и в группе больных без ОХИ (Ме = 3,3 и 4,3 пг/мл против 0,2 пг/мл). Концентрации пяти цитокинов, а именно IL-1â, IL-2, IL-4, IL-10 и INF-ã, не превышали верхней границы референсного диапазона у всех пациентов опытных (больные псориазом) и контрольной групп.</p> <p>Оценка индекса PASI в процессе наблюдения за пациентами в первой и второй группах показала существенно более низкие значения данного показателя на неделях 24 и 52 именно в первой группе (0,2 против 7,0 при <italic>р</italic> = 0,02 и 0,1 против 7,2 при <italic>р</italic> = 0,002). Относительный риск развития рецидива заболевания в случае несанации ОХИ составил 2,4 (95% ДИ от 1,1 до 5,2).</p> <p><bold>Заключение.</bold> Получены данные, свидетельствующие о роли IL-8 и очагов хронической инфекции, с наличием которых ассоциирована его повышенная продукция, в ухудшении течения псориаза с частым развитием рецидивов.
