Surgical removal of facial PG is not always possible due to insufficient thickness of the dermis. Treatment of PG with a pulsed dye laser (PDL) or neodymium laser (Nd:YAG) can achieve a noticeable elimination of the lesion but is reported to be accompanied by such side effects as purpura or scarring. It determines the feasibility of introducing into clinical practice the methods of laser treatment of PG using the radiation of a copper vapor laser (CVL) with a wavelength of 578 nm, which effectively absorbed by the blood.</p> <p><bold>Aim.</bold> The assessment of the clinical efficacy and safety of the CVL treating PG.</p> <p><bold>Methods.</bold> 26 adult patients with PG in various parts of the face, including the lips, limbs, and trunk, were included in this study. PG treatment was carried out in one session with CVL (Yakhroma-Med, FIAN) at an average power of 0.7–1.0 W, at a wavelength of 578 nm, exposure time — 0.2–0.3 s. The diameter of the light spot is 1 mm.</p> <p><bold>Results.</bold> Immediately after the single laser treatment, the PG involved area became grey. In 7–10 days, the irradiated area was utterly similar to the adjacent intact skin. No postoperative bleeding or infection was noted. Side effects included mild skin atrophy. During the follow-up observation for five years, no side effects were found.</p> <p><bold>Conclusion.</bold> The high efficiency of PG elimination using CVL in the absence of pronounced side effects allows suggesting this method for introducing into dermatologists and cosmetologists' clinical practice as a highly effective and inexpensive method of treatment.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> Пиогенная гранулема (ПГ), или лобулярная капиллярная гемангиома, код по МКБ10: L98.07. ПГ проявляется в виде одиночной папулы ярко-красного или фиолетового цвета, размером до 20 мм. ПГ локализуется на лице, пальцах, голове, руках и ногах, в межъягодичной складке, а также, на слизистых полости рта. Хирургическое удаление лицевых ПГ, особенно, в детском возрасте, не всегда представляется возможным из-за недостаточной толщины дермы и риска рубцов. Лечение ПГ с помощью импульсного лазера на красителе и неодимового лазера позволяет добиться элиминации патологического очага, но сопровождается такими побочными эффектами, как пурпура или появление рубцов. Необходимость предотвращения побочных эффектов лечения ПГ определяет целесообразность внедрения в клиническую практику методов оптимального лазерного воздействия без повреждения ретикулярного слоя дермы. Именно такое воздействие обеспечивается лазером на парах меди (ЛПМ), генерирующим излучение с длиной волны 578 нм.</p> <p><bold>Цель исследования.</bold> Оценить клиническую эффективность и риск побочных эффектов удаления ПГ с помощью ЛПМ.</p> <p><bold>Методы.</bold> Лечение ПГ с помощью ЛПМ проведено у 26 пациентов (14 женщин и 12 мужчин) в возрасте от 16 до 62 лет, с локализацией ПГ на веке, кончике или у крыла носа, в периоральной области, на губе, на конечностях и туловище. Для удаления ПГ оказалось достаточным одной процедуры облучения ЛПМ (Яхрома-Мед, ФИАН) с длиной волны 578 нм, средней мощностью ЛПМ 0,7–1,0 Вт, экспозицией 0,2–0,3 с и диаметром светового пятна 1 мм.</p> <p><bold>Результаты.</bold> После одной лазерной процедуры область ПГ приобретала серый оттенок. Через 7–12 дней обработанная область по окраске и структуре не отличалась от окружающей интактной кожи. Не было отмечено послеоперационного кровотечения или инфекции.
