We made the clinical assessment of symptoms of onychodystrophy among patients with and without psoriatic arthritis and the evaluation of the diagnostic significance of the ultrasound method for diagnosing psoriatic arthritis under the control of magnetic resonance imaging of the joints.</p> <p><bold>Results.</bold> 45 patients with psoriatic arthritis and 134 patients with psoriasis without arthritis were examined. Psoriatic onychodystrophy was significantly more common in the group of patients with psoriatic arthritis (<italic>p</italic> = 0.028). The most characteristic symptoms of onychodystrophy in patients with psoriatic arthritis are oil spots and symptom combinations: oil spots and subungual hyperkeratosis, onycholysis and subungual hyperkeratosis, pits and longitudinal ridges, oil spots and longitudinal ridges. The ultrasound method for the diagnosis of psoriatic arthritis has shown its high diagnostic significance (Se = 81.8%, Sp = 88.0%). The most characteristic symptoms of arthritis on ultrasound examination are hypoechoic structures of the Achilles tendons and the presence of fluid in the peri- and retro-Achilles bursa.</p> <p><bold>Conclusions.</bold> The symptom of oil spots and combinations of oil spots with subungual hyperkeratosis, onycholysis with subungual hyperkeratosis, pits with longitudinal ridges and oil spots with longitudinal ridges can be considered diagnostically significant in the examination of early psoriatic arthritis, and the ultrasound method is highly effective in diagnosing early manifestations of arthritis.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> Существует необходимость оценки диагностической значимости симптомов изменений ногтей, а также доступных эффективных методов диагностики псориатического артрита на ранних сроках.</p> <p><bold>Цель исследования. </bold>Изучить встречаемость симптомов псориатической ониходистрофии при псориатическом артрите и оценить диагностическую значимость ультразвукового метода для выявления энтезита как раннего проявления псориатического артрита у пациентов с изменениями ногтевой пластинки и отсутствием диагноза псориатического артрита в анамнезе.</p> <p><bold>Методы. </bold>Проспективное клиническое сравнительное исследование на базе клиники кожных и венерических болезней ВМедА. Клиническая оценка встречаемости симптомов ониходистрофии у пациентов с псориатическим артритом и без него. Определение диагностической значимости ультразвукового метода диагностики псориатического артрита под контролем магнитно-резонансной томографии суставов.</p> <p><bold>Результаты.</bold> Обследовано 45 пациентов с псориатическим артритом и 134 больных псориазом без артрита. Псориатическая ониходистрофия статистически значимо чаще выявлена в группе пациентов с псориатическим артритом (<italic>р</italic> = 0,028). Наиболее характерными симптомами ониходистрофии у больных псориатическим артритом оказались масляные пятна, а также комбинации: масляные пятна и подногтевой гиперкератоз, онихолизис и подногтевой гиперкератоз, точечные вдавления и продольные гребни, масляные пятна и продольные гребни. Ультразвуковой метод диагностики псориатического артрита показал высокую диагностическую значимость (Se = 81,8%, Sp = 88,0%). Наиболее характерными симптомами артрита при ультразвуковом обследовании являются изменения эхоструктуры ахилловых сухожилий (гипоэхогенные структуры) и наличие жидкости в пери- и ретроахилловой сумке.</p> <p><bold>Заключение.</bold> Симптомы масляных пятен, а также комбинации масляных пятен с подногтевым гиперкератозом, онихолизиса с подногтевым гиперкератозом, точечных вдавлений с продольными гребнями и масляных пятен с продольными гребнями можно считать диагностически значимыми при обследовании на псориатический артрит, а ультразвуковой метод — высокоэффективным в диагностике ранних проявлений артрита.</p></trans-abstract><kwd-group xml:lang="en"><kwd>psoriatic onychodystrophy</kwd><kwd>psoriatic arthritis</kwd><kwd>oil spots symptom</kwd><kwd>ultrasound method</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>псориатическая ониходистрофия</kwd><kwd>псориатический артрит</kwd><kwd>симптом масляных пятен</kwd><kwd>ультразвуковая диагностика</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>McGonagle D, Tan AL, Benjamin M.
