Taking into account the reports about a significant role of <italic>Malassezia</italic> in the pathogenesis of a number of dermatoses, the study of this issue is of scientific, practical interest.</p> <p><bold>Aims.</bold> To evaluate the antifungal activity of external forms of activated zinc pyrithione in the treatment of psoriasis, seborrheic dermatitis, pityriasis versicolor.</p> <p><bold>Materials and methods.</bold> An open prospective study was conducted between March and July 2022. Patients with psoriasis, seborrheic dermatitis, pityriasis were treated with external forms of activated zinc pyrithione for 21 days. Skin scales and circular prints from lesion foci, as well as from skin areas without clinical manifestations before and after therapy were studied. A quantitative assessment of skin colonization by micromycetes of <italic>Malassezia</italic> was performed using microscopic, cultural methods of examination. Clinical efficacy and drug safety of the therapy was assessed using the Dermatological Symptom Scale Index, by recording adverse events at weeks 0, 1, 2, 3.</p> <p><bold>Results.</bold> 64 patients aged 18 to 65 years with diagnoses of psoriasis, seborrheic dermatitis, and pityriasis versicolor were included. 60 patients completed the study, 4 were excluded due to failure to adhere to the schedule.</p> <p>In patients with seborrheic dermatitis and pityriasis versicolor in the lesion foci after therapy, a significant decrease colonization level according to the results of microscopic, cultural studies was observed. In psoriasis patients, a significant decrease in the colonization level was obtained only based on the results of microscopic examination.</p> <p>In all groups, significant differences in comparison to the initial level were registered already at the 1st week of treatment. No adverse events were registered.</p> <p><bold>Conclusion.</bold> Activated zinc pyrithione in the form of cream and aerosol showed moderate antifungal activity against micromycetes of the genus <italic>Malassezia</italic>.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> В настоящее время недостаточно данных о противогрибковой активности широко применяющегося в клинической практике активированного цинка пиритиона. С учетом сообщений о значимой роли грибов рода <italic>Malassezia</italic> в патогенезе целого ряда воспалительных дерматозов изучение данного вопроса представляет научный и практический интерес.</p> <p><bold>Цель исследования.</bold> Оценить антифунгальную активность наружных форм активированного цинка пиритиона при лечении псориаза, себорейного дерматита и отрубевидного лишая.</p> <p><bold>Методы. </bold>Проведено открытое проспективное клинико-лабораторное исследование в период с марта по июль 2022 г. Больные псориазом, себорейным дерматитом и отрубевидным лишаем получали лечение наружными формами активированного цинка пиритиона в течение 21 дня. Изучались кожные чешуйки и круговые отпечатки из очагов поражения, а также с участков кожи без клинических проявлений до и после проведенной терапии. Проводилась количественная оценка колонизации кожи микромицетами рода <italic>Malassezia</italic> с использованием микроскопического и культурального методов исследования. Дополнительно оценивались клиническая эффективность проведенной терапии с применением дерматологического индекса шкалы симптомов (ДИШС), а также безопасность препарата путем регистрации нежелательных явлений на неделях 0, 1, 2 и 3.</p> <p><bold>Результаты.</bold> В исследование были включены 64 пациента в возрасте от 18 до 65 лет с диагнозами ограниченный псориаз (16 мужчин, 8 женщин), себорейный дерматит (20 мужчин) и отрубевидный лишай (19 мужчин, 3 женщины).</p> <p>У больных себорейным дерматитом и отрубевидным лишаем в очагах поражения после проведенной наружной терапии отмечено значимое уменьшение уровня колонизации по результатам микроскопического и культурального исследований. У больных псориазом значимое снижение уровня колонизации получено только по результатам микроскопического исследования.</p> <p>Во всех группах при оценке ДИШС значимые различия в сравнении с исходным уровнем (неделя 0) получены уже на 1-й неделе терапии (<italic>p</italic> &lt; 0,001). Нежелательных явлений, связанных с применением препарата, зарегистрировано не было.</p> <p><bold>Заключение.</bold> Активированный цинк пиритион в форме крема и аэрозоля показал умеренную антифунгальную активность в отношении микромицетов рода <italic>Malassezia</italic>.
