Atopic dermatitis (AD) is one of the most common inflammatory skin diseases, affecting up to 20.0% of children and 2.0–8.0% of adults worldwide. Patients with atopic dermatitis usually have dry skin and itching, they are at higher risk of developing asthma, as well as allergic rhinitis. Features of the clinical course of AD are associated with both interaction of genes and influence of environmental factors. Studying epigenetic mechanisms could provide understanding of the most likely mechanisms by which environmental factors influence gene expression and contribute to the development of AD. The aim of the study is to conduct a comprehensive molecular genetic analysis (assessment of the level of global DNA methylation and expression of the filaggrin gene (<italic>FLG</italic>) in patients with moderate to severe AD. Material and methods: The case-control study was conducted from January 2022, through June 2022. A total of 32 patients with AD and 6 healthy volunteers were recruited. The level of global DNA methylation in the blood of patients with AD and healthy individuals was assessed. The <italic>FLG</italic> expression was measured by polymerase chain reaction in the leukocyte fraction of blood. A three-stage cycle is used during PCR. To determine the expression level of the <italic>FLG </italic>gene, total RNA was isolated, followed by a reverse transcription, amplification, and detection reaction.</p> <p><bold>Results</bold>. A hypermethylation of the genome in patients with AD is 2.3 times higher than the methylation of genome in controls (<italic>p</italic> = 0.003),it was also found that the expression level of the <italic>FLG</italic> gene (exon 3, position 7–8, and exon 1) in the leukocyte fraction of blood in patients with AD did not significantly differ from the control group (<italic>p</italic> = 0.41).</p> <p><bold>Conclusion</bold>. The current study found that the epigenome of AD patients differs from healthy individuals. The study of pathogenetic mechanisms is the basis for the development of new methods of targeted therapy for this socially significant dermatosis.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование</bold>. Атопический дерматит (АД) — одно из наиболее распространенных воспалительных заболеваний кожи, которое поражает до 20,0% детей и 2,0–8,0% взрослых по всему миру. У пациентов с АД обычно наблюдаются сухость кожи и зуд, они подвержены более высокому риску развития астмы, а также аллергического ринита. Особенности клинического течения АД связаны как с взаимодействием генов, так и с влиянием факторов окружающей среды. Понять наиболее вероятные механизмы, с помощью которых факторы окружающей среды влияют на экспрессию генов и способствуют развитию АД, можно с помощью изучения эпигенетических механизмов.</p> <p><bold>Цель исследования</bold> — проведение комплексного молекулярно-генетического анализа (оценка уровня глобального метилирования ДНК и экспрессии гена филаггрина (<italic>FLG</italic>) у пациентов с АД среднетяжелого и тяжелого течения.</p> <p><bold>Методы</bold>. Исследование «случай–контроль», проведенное в период с января по июнь 2022 г., включало 32 пациента с АД и 6 здоровых добровольцев. Произведена оценка уровня глобального метилирования ДНК в крови пациентов с АД и здоровых лиц. Методом полимеразной цепной реакции в лейкоцитарной фракции крови был определен уровень экспрессии гена <italic>FLG</italic>. В ходе ПЦР применялся трехэтапный цикл. Для определения уровня экспрессии гена <italic>FLG</italic> выделяли тотальную РНК с последующей реакцией обратной транскрипции, амплификации и детекции.</p> <p><bold>Результаты</bold>.<bold> </bold>Установлено, что у пациентов с АД наблюдается гиперметилирование генома, в 2,3 раза превышающее метилирование пациентов группы контроля (<italic>p</italic> = 0,003), также установлено, что уровень экспрессии гена <italic>FLG</italic> (экзон 3, позиция 7–8, и экзон 1) в лейкоцитарной фракции крови у пациентов с АД статистически значимо не отличается от контрольной группы (<italic>p</italic> = 0,41).</p> <p><bold>Заключение</bold>. Установлено, что эпигеном больных АД отличается от эпигенома здоровых лиц.
