An important condition for achieving optimal therapeutic effect is the identification of factors that can impact on the efficacy of therapy and considering their influence when prescribing treatment.</p> <p><bold>Aims.<italic> </italic></bold>The present study aimed to identify the factors which affect the efficacy of narrow-band phototherapy in patients with atopic dermatitis</p> <p><bold>Methods.<italic> </italic></bold>A prospective, open-label trial was conducted to evaluate the efficacy and safety of narrow-band UVB phototherapy for patients with moderate-to-severe atopic dermatitis. All patients were treated with narrow-band UVB phototherapy four times weekly for 5 weeks. Disease severity was evaluated by SCORing of the Atopic Dermatitis Index (SCORAD) and Eczema Area and Severity Index (EASI). Distribution of patients by the severity of therapeutic effect was evaluated. To compare the efficacy of therapy depending on initial atopic dermatitis severity, initial and cumulative irradiation doses, skin phototype, and smoking status patients were divided into subgroups.</p> <p><bold>Results.</bold> 40 patients with moderate-to-severe atopic dermatitis received course of narrow-band UVB phototherapy. After NB-UVB therapy SCORAD and EASI scores reduced from 45.6 ± 11.4 at baseline to 22.6 ± 12.4 (<italic>p</italic> &lt; 0,05) and from 14.4 ± 7.2 at baseline to 4.1 ± 3.9 (<italic>p</italic> &lt; 0,05) respectively demonstrating the efficacy of narrow-band UVB phototherapy in patients with atopic dermatitis. Our investigation showed that tobacco smokers had definitely lower efficacy of NB-UVB phototherapy in comparison with non-smokers. Narrow-band UVB phototherapy had definitely higher efficacy when it is started with an initial dose 0.2–0.3 J/cm<sup>2</sup> chosen in compliance with results of MED determing in comparison with an initial dose 0.05–0.15 J/cm<sup>2</sup> selected according to skin phototype.</p> <p><bold>Conclusions.<italic> </italic></bold>Factors that impact on the efficacy of narrow-band UVB phototherapy in patients with atopic dermatitis were identified. It was determined that using higher initial dose is associated with higher efficacy of therapy. The obtained data suggest the opportunity of decrease in efficacy of therapy in smokers with atopic dermatitis.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование. </bold>Узкополосная средневолновая ультрафиолетовая терапия характеризуется вариабельностью выраженности терапевтического эффекта при использовании в лечении больных атопическим дерматитом. Условием достижения оптимального терапевтического эффекта является определение факторов, способных повлиять на эффективность лечения, и учет их влияния при назначении терапии.</p> <p><bold>Цель. </bold>Определить факторы, влияющие на эффективность узкополосной фототерапии больных атопическим дерматитом.</p> <p><bold>Методы. </bold>Проведено открытое, проспективное клиническое исследование эффективности и безопасности узкополосной фототерапии больных атопическим дерматитом средней и тяжелой степени тяжести. Курс узкополосной средневолновой ультрафиолетовой терапии проводили в режиме: 4 процедуры в неделю в течение 5 недель. Эффективность терапии оценивали с помощью индексов EASI и SCORAD. Оценивали распределение пациентов по степени выраженности терапевтического эффекта. Разделяли пациентов на подгруппы для сравнения эффективности лечения в зависимости от исходной тяжести атопического дерматита, фототипа кожи, величин начальной и кумулятивной доз облучения, курения пациентами табака.</p> <p><bold>Результаты. </bold>Курс узкополосной фототерапии проведен 40 пациентам с атопическим дерматитом средней и тяжелой степени тяжести. Индексы EASI и SCORAD уменьшились в процессе лечения соответственно с 14,4 ± 7,2 до 4,1 ± 3,9 балла (<italic>p</italic> &lt; 0,05) и с 45,6 ± 11,4 до 22,6 ± 12,4 балла (<italic>p</italic> &lt; 0,05), что показало эффективность лечения пациентов с атопическим дерматитом методом узкополосной фототерапии. Обнаружено, что выраженность терапевтического эффекта узкополосной фототерапии меньше у пациентов, курящих табак, по сравнению с некурящими пациентами. Эффективность узкополосной фототерапии была более высокой при назначении ее по результатам определения МЭД в дозе 0,2–0,3 Дж/см<sup>2</sup> по сравнению с начальной дозой в соответствии с фототипом кожи в дозе 0,05–0,15 Дж/см<sup>2</sup>.</p> <p><bold>Заключение. </bold>Идентифицированы факторы, влияющие на эффективность узкополосной фототерапии больных атопическим дерматитом. Установлено, что применение более высокой начальной дозы облучения ассоциировано с более высокой эффективностью терапии.
