most common type of pathological hair loss in women is androgenetic alopecia (AGA), arises due to hormonal and micronutrient imbalances. During the COVID-19 epidemic, there has been an increase in the number of female patients with alopecia as a complication, with some individuals experiencing alopecia the sole indication of asymptomatic COVID-19.</p> <p><bold>Aims</bold><bold>.</bold><italic> </italic>The search for objective criteria for the differential diagnosis of AGA and post-COVID alopecia in women based on trichological and laboratory markers.</p> <p><bold>Methods</bold><bold>.</bold> The including criteria for AGA were elevated dihydrotestosterone (DHT) levels, for the post-COVID alopecia — a diagnosis of COVID-19 using RT-PCR and the presence of alopecia symptoms for both conditions. Quantitative characteristics of hairs were analyzed based on trichogram and phototrichogram. Levels of DHT, vitamins B<sub>9</sub>, B<sub>12</sub>, D, E, Ca, Fe, Mg, Se, Cu, and Zn were evaluated in serum. CART algorithm (Classification and Regression Trees) was applied to determine criteria to differentiate between patients with androgenetic and post-COVID alopecia.</p> <p><bold>Results</bold><bold>.<italic> </italic></bold>Analysis revealed the change in telogen/anagen ratio in the androgen-dependent zone in in AGA, and in androgen-dependent (parietal) area in post-COVID. Notably, patients with post-COVID alopecia exhibited elevated DHT levels compared to reference, with no significant difference in comparison to AGA. There was a significant 46.4% reduction in Cu content (<italic>p</italic> = 0.006) alongside an 24.7% increase in Se levels (<italic>p</italic> = 0.003) in post-COVID alopecia.</p> <p><bold>Conclusion</bold><bold>.<italic> </italic></bold>The percent of telogen hair and serum Se level as the objective criteria for the differential diagnosis of AGA and post-COVID alopecia in women are presented.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование</bold><bold>. </bold>Наиболее частой формой патологической утраты волос является андрогенная алопеция (АГА), возникающая на фоне гормональных и микронутриентных нарушений. При COVID-19 резко возросло количество пациенток с осложнением в виде постковидной алопеции (ПКА), зачастую при отсутствии других симптомов COVID-19.</p> <p><bold>Цель исследования. </bold>Поиск объективных критериев дифференциальной диагностики АГА и ПКА у женщин, основанных на трихологических и лабораторных показателях.</p> <p><bold>Методы. </bold>Критерии формирования групп: АГА — повышенный уровень дигидротестостерона, наличие клинических признаков АГА; ПКА — ранее подтвержденный РТ-ПЦР диагноз COVID-19, наличие признаков алопеции. Характеристика волосяного покрова проводилась на основе данных трихограмм и фототрихограмм. В сыворотке крови пациенток определялись дигидротестостерон, витамины B₉, B₁₂, D и Е, а также Ca, Fe, Mg, Se, Cu, Zn. Для определения критериев дифференцирования АГА и ПКА был применен алгоритм CART с формированием древовидной иерархической структуры.</p> <p><bold>Результаты. </bold>Анализ трихограмм и фототрихограмм показал, что если при АГА изменяется соотношение телоген/анаген в андрогензависимой (теменной) зоне, то при ПКА — в теменной и затылочной. Парадоксальным результатом стало отсутствие различий в уровне дигидротестостерона между группами АГА и ПКА. Выявлено снижение Cu (на 46,4%; <italic>р</italic> = 0,006) и повышение Se (на 24,7%; <italic>р</italic> = 0,003) при ПКА в сравнении с АГА. Алгоритм CART позволил с 96%-й вероятностью разделить случаи АГА и ПКА с помощью таких дифференцирующих критериев, как доля волос в фазе телогена в теменной зоне скальпа (&lt; 17% — АГА; ≥ 17% — ПКА) и уровень микроэлемента Se в плазме крови (&lt; 107 мкг/л — АГА; ≥ 107 мкг/л — ПКА).</p> <p><bold>Заключение. </bold>Представлены объективные критерии дифференциальной диагностики АГА и ПКА в виде доли волос в фазе телогена в теменной зоне скальпа и уровня Se в плазме крови.</p></trans-abstract><kwd-group xml:lang="en"><kwd>female androgenic alopecia</kwd><kwd>post-COVID-19 alopecia</kwd><kwd>trichogram</kwd><kwd>dihydrotestosterone</kwd><kwd>vitamins</kwd><kwd>trace elements</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>андрогенная алопеция у женщин</kwd><kwd>алопеция после перенесенного COVID-19</kwd><kwd>трихограмма</kwd><kwd>дигидротестостерон</kwd><kwd>витамины</kwd><kwd>микроэлементы</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Ramos PM, Miot HA.
