A prospective open, simple, comparative, scientific study included 101 patients (men — 33, women — 68) with bullous dermatoses. The study was conducted from 2017 to 2023.</p> <p><bold>Results</bold><bold>.</bold> Statistically significant differences were revealed for a number of HLA class II indicators. Carriers of HLA-DRB1*3, DRB1*4, DRB1*14, DRB1*16, DQB1*0304, DQB1*0502-4, DQB1*0503, DQB1*02 and DQA1*0301 should be identified as a risk group for the development of pemphigus vulgaris, benign familial pemphigus and bullous pemphigoid. Patients carrying histocompatibility gens HLA-DRB1*15, DRB1*17, DQB1*201, DQB1*303, DQB1*602-8 have increased resistance to the above-mentioned bullous dermatoses.</p> <p><bold>Conclusions</bold><bold>.</bold> An association was found between histocompatibility gens HLA class II, pemphigus vulgaris, bullous pemphigoid and benign familial pemphigus. The data obtained can be used to predict the development of the above-mentioned diseases, develop a set of preventive recommendations, verify the correct diagnosis in the early stages of diseases.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование</bold><bold>.</bold> Пузырные дерматозы — группа тяжелых гетерогенных заболеваний, потенциально опасных для жизни и в значительной степени ухудшающих ее качество. Раннее выявление у пациентов с отягощенным по пузырным дерматозам наследственным анамнезом предикторов развития данной патологии посредством использования HLA-диагностики позволит лечащему врачу разработать комплекс профилактических рекомендаций, верифицировать правильный диагноз на ранних стадиях заболевания и минимизировать риски триггерного влияния экспосом-факторов.</p> <p><bold>Цель исследования. </bold>Установить ассоциативную связь генов гистосовместимости HLA II класса с пузырными дерматозами на примере вульгарной пузырчатки, буллезного пемфигоида и доброкачественной семейной пузырчатки.</p> <p><bold>Методы.</bold> В проспективное открытое, простое сравнительное научное исследование был включен 101 пациент (мужчины — 33, женщины — 68) с пузырными дерматозами (вульгарная пузырчатка, буллезный пемфигоид, доброкачественная семейная пузырчатка). Исследование проводилось с 2017 по 2023 г. Осуществляли типирование генов гистосовместимости HLA методом полимеразной цепной реакции с набором специфичных для последовательностей праймеров, типировали гены гистосовместимости HLA II класса (DRB1, DQA1, DQB1).</p> <p><bold>Результаты.</bold> Выявлены статистически значимые различия для ряда показателей HLA II класса. Носителей генов гистосовместимости HLA-DRB1*3, DRB1*4, DRB1*14, DRB1*16, DQB1*0304, DQB1*0502-4, DQB1*0503, DQB1*02 и DQА1*0301 следует отнести в группу риска по развитию вульгарной пузырчатки, буллезного пемфигоида и доброкачественной семейной пузырчатки.
