Recently, various methods of molecular medicine have been actively developed, however, targeted therapy deserves special attention, which consists of chemical agents that have specific target as a specific protein or enzyme. Targeted therapy is a promising direction in many branches of medicine, especially in dermatology.</p><p>Despite the wide range of biological products, their use may be accompanied by an increased risk of infectious processes and malignant neoplasms, which makes the search for a new pharmacological solution in targeted therapy even more relevant.</p><p>This review presents the possibilities and prospects for the therapeutic use of the phosphodiesterase-4 inhibitor from the group of small molecules — apremilast, primarily in the treatment of psoriasis and psoriatic arthritis.</p></abstract><trans-abstract xml:lang="ru"><p>Лечение псориаза и псориатического артрита, особенно среднетяжелого и тяжелого течения, представляет трудности. В последнее время активно развиваются различные методы молекулярной медицины, однако особого внимания заслуживает таргетная терапия, заключающаяся в использовании химических агентов, направленных конкретно на определенный белок или фермент. Таргетная терапия является перспективным направлением во многих отраслях медицины, особенно в дерматологии.</p><p>Несмотря на широкий спектр возможностей биологических препаратов, их применение может сопровождаться повышением рисков возникновения инфекционных процессов и злокачественных новообразований, что делает поиск новых фармакологических решений в таргетной терапии еще более актуальным.</p><p>В настоящем обзоре представлены возможности и перспективы терапевтического применения ингибитора фосфодиэстеразы-4 из группы малых молекул — препарата апремиласт, прежде всего при лечении псориаза и псориатического артрита.</p></trans-abstract><kwd-group xml:lang="en"><kwd>psoriasis</kwd><kwd>psoriatic arthritis</kwd><kwd>small molecules</kwd><kwd>phosphodiesterase-4 inhibitor</kwd><kwd>apremilast</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>псориаз</kwd><kwd>псориатический артрит</kwd><kwd>малые молекулы</kwd><kwd>ингибитор фосфодиэстеразы-4</kwd><kwd>апремиласт</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>1.
