Иммунные механизмы псориаза. Новые стратегии биологической терапии

Аннотация
Современные научные данные указывают на то, что псориаз является иммунозависимым заболеванием с генетической предрасположенностью к его развитию. Важную роль в развитии псориаза играют дендритные клетки и активированные Т-лимфоциты, взаимодействие которых запускает ряд механизмов, приводящих в конечном итоге к развитию воспалительного процесса и формированию псориатических поражений кожи. Посредниками в данных механизмах выступают секретируемые иммунокомпетентными клетками цитокины, такие как IL-12, IL-23, IFN-y и TNF-a. Новейший терапевтический подход к лечению псориаза состоит в создании и введении в организм больного специфических моноклональ-ных антител к патогенетически значимым цитокинам. В настоящее время существуют препараты, успешно блокирующие развитие псориатических поражений кожи путем специфического связывания цитокинов IL-12, IL-23 и TNF-a. Препараты, названные устекинумаб и ABT-874, подтвердили свою терапевтическую активность в отношении псориаза во 2-й и 3-й фазах клинических исследований. Препарат устекинумаб в ходе данных исследований также продемонстрировал сравнимый с плацебо профиль безопасности.Установлено, что с высокой интенсивностью ответа больного псориазом на лечение препаратом инфликсимаб ассоциировался гомозиготный ТТ генотип гена TNF-R-II в 676-м положении и высокий уровень IL10 в сыворотке крови (> 2,7 пг/мл); с ослабленным ответом или отсутствием ответа на лечение препаратом инфликсимаб ассоциировался гомозиготный GG генотип гена TNF-R-II в 676-м положении и низкий уровень IL10 в сыворотке крови (< 1,0 пг/мл). Полученные результаты послужили основанием для разработки метода прогнозирования эффективности лечения больных псориазом генно-инженерным биологическим препаратом инфликсимаб. Получены пилотные результаты, свидетельствующие о возможной взаимосвязи между составом протеома кожи и терапевтическим ответом больного псориазом на лечение препаратом инфликсимаб.
