Abstract

Androgenetic alopecia (AGA) is the most common reason for hair loss. Its frequency in Caucasian population is up to 80% in men and 42% in women. Current gold standard for AGA treatment includes minoxidil and/or finasteride. Both drugs have moderate treatment efficacy and can cause significant side effects. Minoxidil is the only option for treatment of AGA in women since finasteride has no efficacy. This review considers actual concepts of molecular mechanisms of pathogenesis and current treatment options of AGA with their limitations and shortcomings. Current state and perspectives of novel approaches to AGA treatment, potentially more effective and safe than minoxidil and finasteride, are described. The review discusses growth factors and cytokines, topical 5-alpha-reductase inhibitors, androgen receptor antagonists, prostaglandin analogs and antagonists, Wnt signaling activators and platelet-rich plasma injections.

Reasonable antimicrobial therapy in patients with pyodermatosis

Abstract

The data are given about the history of the invention and development of antimicrobial therapy in the XX-th and the beginning of the XXI-th centuries. There are presented data on the pharmacological effects of antibacterial agents from different groups that are used for the treatment of patients with pyodermatosis, dose schedules, pharmacokinetics, adverse effects, contraindications, and medicine interactions. The main causes leading to the formation of resistant microorganism strains are pointed. Advances of broad-spectrum antibacterial antibiotics for treatment the pyodermic patients are demonstrated on the example of amoxicillin+clavulinic acid, josamycin and doxycycline monohydrate. The arguments justifying the need to refuse empiric administration of antibacterial therapy are presented. To select a rational causal initial therapy of patients with pyoderma it is necessary to use the results of microbiological study of etiology and pathogenic microflora sensitivity to antimicrobial preparations.

Skin lesion at acute monoblastic leukemia (clinical observation)

Abstract

At present diagnosis of acute leukemia does not represent any difficulties and is based on morphological and immunophenotypical bone marrow studies. The paper presents a non-standard verification algorithm of acute monoblastic leukemia in elderly female patient, with leukemic skin lesion. Morphologic and immunohistochemical studies of biopsy materials of skin, lymph node and bone marrow allow establishing a linear membership of tumor cells and promptly verifying the diagnosis of leukemia. Lack of the overall clinical examination in such cases leads to diagnostic errors and to late verification of the true diagnosis and significantly worsens the prognosis.

Generalized eruptive histiocytoma: case report

Abstract

The case of generalized eruptive histiocytoma (GEH) in woman of 49 years old is reported. GEH is a rare disease relating to non-langerhans cell histiocytoses which are based on a benign reactive proliferation of histiocytes with immunophenotype inconsistent with Langerhans cells. As a result of treatment with methotrexate and betamethasone the improvement in skin condition was achieved.

The effect of ultraviolet on direct current potentials of the human skin

Abstract

Aim of the study. investigation of the suntan on the level of omnipresent electrical potential of skin in frames of development of the hypothesis about the melanocyte participation in skin electrical activity formation. Material and methods. 11 men and 11 women on summer holyday took part in investigation. The relationship between the levels of skin omnipresent electrical potential and the reflection factor were investigated. The omnipresent electrical potentials were registered as the potential differences between the two areas on skin using the Orion 261S pH meter and the liquid Ag/AgCl electrodes of the same company. The reflecting factor was estimated with the help of the spectroradiometer LI-1800 (“LI-COR”) using the included intgrational sphere 1800-12 coated with barium sulfate. Hole size was 1,45 cm. The reflection factor was estimated at the wave band 375-800 nanometers with the scanning pitch 1 nm. Main results. There is a mathematical relation between the magnitudes of potential differences and the reflection factors of the areas of sunburnt skin and the areas of the skin not affected with sun at wave light 620-729 nm. The relationship could be approximated in linear equation. Spearman correlation factor between these two parameters was -0,43 (p = 0,045), between the differences in levels of skin omnipresent electrical potential and the reflection factors of the sunburnt skin was r = -0,52 (p = 0,01), between the differences of the electrical potentials of sunburnt skin and the skin not affected with sun was r = -0,28 (p = 0,2). The presented results of investigation and the data from literature allow to conclude that there is a positive correlation between the level of omnipresent electrical potential of skin and the quantity of melanosomes filled with melanin. Conclusion. It is suggested that melanocytes take part in formation of the skin electrical activity via melanosomes filled with melanin. At the same time the melanocyte contribution in skin electrical activity is not assessed.

Methotrexate: Revisited efficiency and safety of drug administration in psoriasis patients

Abstract

The article presents the current data of the literature on methotrexate, which is now one of the most commonly used preparation for the systemic treatment of patients with moderate to severe psoriasis. The following problems are under consideration: estimation by specialists of response to systemic psoriasis therapy and possible therapeutic strategies; selecting initial doses of methotrexate for the treatment of patients with psoriasis; the possibilities of combined use with genetically engineered biological agents and monitoring of therapy. The data from randomized clinical trials on the long-term continuous treatment with methotrexate (efficacy, safety); methods of its administration to patients and time and criteria for long-term effecasy are reported. There are presented the data on the mechanisms of methotrexate action and the new data about the impact on the adenosine metabolism and the ability of the preparation to modulate the inflammatory response in the skin of patients by inhibiting the cellular components of the inflammatory infiltrate in the skin (dendritic antigen-producing cells and T-lymphocytes), as well as the suppression of expression of some proinflammatory cytokines (IFN-y and IL17A).
