Аннотация
Гнойный гидраденит (ГГ) — хроническое рецидивирующее заболевание кожи, которое характеризуется развитием аутоиммунного воспаления инфундибулярной части волосяного фолликула и апокриновых потовых желез, расположенных в подмышечных впадинах, паховой, аногенитальной областях, в субмаммарных складках, области около ареол молочных желез и околопупочной зоне. ГГ тяжело поддается терапии и оказывает крайне негативное влияние на качество жизни. Это заболевание выделено в отдельную нозологию, принципиально отличающуюся по этиопатогенезу от острого бактериального гидраденита. Точная распространенность ГГ неизвестна, однако она сильно варьирует в различных исследованиях, составляя в общей популяции от 0,03 до 4%. Обнаружение ИЛ-1β, ИЛ-23 и ИЛ-17 в очагах поражения указывает на ключевую роль аутоиммунного воспаления с участием Th17-лимфоцитов в патогенезе ГГ. Факторами риска при этом также являются курение, ожирение, механическое трение, генетическая предрасположенность. Для постановки диагноза необходимо наличие трех обязательных критериев: типичная локализация, типичные высыпания и хроническое рецидивирующее течение. Не существует «золотого стандарта» в оценке тяжести заболевания, так как все имеющиеся системы оценки имеют множество ограничений, поэтому в настоящее время нерешенными остаются многие вопросы в отношении этого заболевания — от терминологии и четких, максимально объективных оценочных критериев тяжести процесса до разработки отечественных клинических рекомендаций.
