Comparative characteristics of the results of fractional photothermolysis used for neck skin of women in different age groups

Abstract

The purpose of this research is scientific rationale for fractional photothermolysis usage to correct age-related changes of neck skin of women in different age groups. Materials and Methods. A comparative study of the results for fractional photothermolysis (FP) treatment in order to correct involutional changes in neck skin of 60 women in different age groups has been carried out (first group - 40-49 years old, second group - 50-60 years old). Skin moisture, its smoothness (relief), the width of the mouths of the pilosebaceous unit, the severity of pigmentation, the depth and width of wrinkles before the treatment, 1 week after and 1 month after the procedure have been investigated. Main results. After a week of having FP it affected all investigated functional skin parameters more in the 2nd age group than in the 1st one. Decreasing in moisture and smoothness of the skin, increasing the width and the depth of wrinkles, pores and pigmentation width have been marked. In one month there was more significant positive dynamics in parameters of smoothness, wrinkles width in the 2nd group (p < 10-8, p < 10-7) than in the 1st group (p = 0.002, p < 10-5). Hydration of the skin, the width of the mouths of the pilosebaceous unit, the depth of wrinkles and pigmentation changed more significantly among patients in group 1 (p < 10-7, p = 0.001, p < 10-5, p < 10-8). Conclusion. Skin functional parameters of patients from the first group deteriorated significantly less in comparison with the second group one week after PF procedure was carried out. 1 month after the 1st group is indicated with significant improvement in moisture, the width of the pilosebaceous unit, the depth of wrinkles and pigmentation, and the 2nd group is indicated with improvement of smoothness and width of wrinkles. The differences discovered in skin condition dynamics in different age groups should be considered when planning the aesthetic outcome of FP procedure.

Abstract

The article describes a clinical case of a combination of Crocker-Adamson follicular spinulosa, lichen acuminatus and Little-Lassueur syndrome in a young man aged 17. The patient had the onset of atopic manifestations at an early age (atopic dermatitis, rhinitis, conjunctivitis) along with disorders related to the process of keratosis. The article provides current data on the etiology, pathogenesis and treatment of these diseases.

Current methods of non-invasive diagnostics of skin melanoma

Abstract

Malignant skin neoplasms including melanoma are now occupying the leading positions in the list of cancer pathologies in Russia, a number of European countries and United States. The percentage of melanoma is less than 4% of the total number of skin neoplasms but melanomas account for up to 80% of lethal cases caused by malignant skin neoplasms. Due to the aggressive nature of melanomas including the proliferative reaction to any invasions including diagnostic ones, there is an urgent need in a reliable non-invasive method to differentiate melanomas from other skin neoplasms and diagnose it at an early stage. This review article discloses descriptions, potential, advantages and shortcomings of non-invasive melanoma diagnostics methods: dermatoscopy, SIAscopy and confocal microscopy. Based on the results of the review, the authors made a conclusion that the development of an algorithm of diagnostics taking into account individual clinical and medical history data, results of non-invasive visualization methods and laboratory test results (proteome analysis, tumor markers) would help to overcome the shortcomings of each individual method, diagnose melanomas at early stages and, respectively, personalize the approach to management for each patient.

Current methods of treatment of true acantholytic pemphigus

Abstract

Major stages of the pathogenesis of true acantholytic pemphigus are interpreted in terms of autoimmune pathology; therefore, treatment of this group of diseases remains pathogenetic and is aimed at suppressing the synthesis of autoantibodies against keratinocyte proteins. Treatment with the use of systemic glucocorticosteroids is currently the main method of treatment for true acantholytic pemphigus. To reduce the course dose of glucocorticosteroids, decrease the risk of adverse effect development and achieve long-term remission, further studies of disease pathogenesis and development of new treatment methods for reducing the doses of glucocorticosteroids are of great importance.

Acute inflammation phase proteins in cases of IgE-mediated and IgE-independent atopic dermatitis

Abstract

Goal. To determine the blood serum level of α2-macroglobulin (α2-MG), α1-antitrypsin (α1-AT), lactoferrin (LF) and albumin in patients with IgE-mediated and IgE-independent atopic dermatitis (AD). Materials and methods. The authors tested blood serum of 60 adult patients suffering from atopic dermatitis at the exacerbation stage (30 patients with IgE-mediated and 30 patients with IgE-independent atopic dermatitis) and 20 healthy donors in order to determine the level of these proteins by rocket immunoelectrophoresis, enzyme-linked immunosorbent assay and immunoturbidimetry methods. Major results. The albumin level is reliably reduced in case of IgE-mediated AD while the LF and α1-AT levels are increased, and concentrations of LF α1-AT and a2-MG are increased as compared to healthy people. There are differences between the level of LF, α2-MG and α1-AT. There was a statistically significant correlation between the LF levels and SCORAD score in both forms, and α2-MG and α1-AT only in case of IgE-independent AD. Conclusion. These results confirm the difference in the roles of these polyfunctional proteins in the pathogenesis of IgE-mediated and IgE-independent AD.

Particular features of lipid metabolism in psoriasis patients with concomitant metabolic syndrome

Abstract

Goal. To assess particular features of lipid metabolism in psoriasis patients with concomitant metabolic syndrome (MS). Materials and methods. Total cholesterol, triglycerides (TG) and high density lipoprotein cholesterol (HDL cholesterol) were determined in the blood serum of psoriasis patients by the enzymatic method using a spectrophotometer at the wavelength of 500 nm. The level of low-density lipoprotein cholesterol (LDL cholesterol) was determined by the calculation method using W. Friedewald’s formula (1972). The cholesterol atherogenicity ratio (CAR) was calculated by the formula: CAR = (Total cholesterol - HDL cholesterol) / HDL cholesterol Results. Patients suffering from psoriasis vulgaris are distinguished by lipid metabolism abnormalities characterized by the development of atherogenic dyslipidemia, increase in the total cholesterol, TG and LDL cholesterol in blood, reduction in the HDL cholesterol level and increased CAR. Patients with concomitant metabolic disorders have more obvious changes.

Abstract

Goal. To compare approaches taken by dermatologists and cosmetologists to management of patients suffering from acne taking into consideration the clinical picture of the disease and patient’s age and sex, and to assess the efficacy and safety of an external therapy with azelaic acid in acne patients. Materials and methods. A study conducted by the MIR-O-MED Clinic involved 300 patients aged 14-48 with different forms of acne of different severity. To analyze particular features of the clinical picture of acne as well as age and sex differences in acne patients consulting dermatologists and cosmetologists, researchers collected statistics on the basis of primary medical documents, and then grouped the data by principal and additional indices. Both dermatologists and cosmetologists recommended azelaic acid in the form of a monotherapy and in a combination with cosmetic products as first-line treatment of primary acne patients. The efficacy and safety of an external therapy of acne patients with azelaic acid was assessed. Results. Clinical recovery and considerable clinical improvement was achieved in 94% of the patients who received an external therapy with azelaic acid. Conclusion. A lack of contradictions in coordinated measures taken by cosmetologists and dermatologists predetermines the correct order and combination of basic treatment measures involving tolerable and timely cosmetic correction, which is a prerequisite for solving the acne problem. Azelaic acid used by these categories of physicians for treatment of acne patients substantially improves the quality and efficacy of therapeutic measures.

Topical steroids in the current clinical practice

Abstract

The article discusses issues related to current criteria for selection of glucocorticosteroids for external use as the basic therapy for a great number of allergic and inflammatory skin diseases. The authors emphasize that non-fluorinated GCSs having the best efficacy-to-safety ratio must be the drugs of first choice. The article provides data on a positive clinical experience of using a non-halogenated glucocorticosteroid for external use - hydrocortisone 17-butyrate (Laticort) - for treatment of steroid-sensitive dermatoses in children and adults. The drug has a high anti-inflammatory action and minimum risk of the development of side effects, which is sufficient for using it in sensitive areas of skin (face, neck, folds, genitals) both in children and in adults. The availability of three forms of the drug (solution, cream and ointment) ensures the expedience and convenience of its application at any stage of the inflammatory process and for any localization.

An effectiveness study of Bariederm cream in combined therapy of dermatoses

Abstract

Goal. The aim of the study is evaluation of the effectiveness of a barrier cream Bariederm in combined therapy of dermatoses. Materials and Methods. 3 groups of patients were examined: 32 patients with allergic contact dermatitis, 22 female patients with cold and wind induced dermatitis and 23 children aged 5 years and under with cutaneons mastocytosis. All of them were treated with Bariederm cream. Results. Including of Bariederm cream in combined treatment fetched to increase of clinical efficiency in treatment of dermatoses. Bariederm cream is also well tolerated.
