Current approaches to diagnostics of mycosis fungoides
Abstract
The authors describe strong and weak sides of current methods for diagnostics of mycosis fungoides.
Diagnostics of mycosis fungoides is mainly based on the clinical presentation of this disease, which is a significant problem for
a dermatovenerologist, especially at early stages of the disease when rashes are of different types. Laboratory methods used to
confirm the diagnosis are microscopic examination and immunophenotypic analysis of skin biopsy samples from the affected
region as well as determination of clonality of T-lymphocytes in the skin using the PCR method.

Tacrolimus: 10 years of successful clinical application
Abstract
Tacrolimus (FK506) is the first drug from the new class of drugs - calcineurin inhibitors. It has a great potential and can change
the current approaches to treatment of different dermatoses. Topical forms of Tacrolimus are activated due to the effect on T-lymphocyte
signal transduction, and they also inhibit gene transcription. As a result, the T-cell response to antigens is alleviated.
Percutaneous absorption of Tacrolimus in affection foci is higher than in healthy skin. Due to this a smaller amount of the drug
is absorbed by the skin as long as the skin is healed. Systemic absorption of Tacrolimus is low in the course of treatment. The
maximum clinical experience has been gained in the field of applying Tacrolimus for treatment of atopic dermatitis. In a number of
clinical studies, 0.03-0.3% Tacrolimus ointment efficiently reduced the severity of symptoms and course of atopic dermatitis in
adults and children. Moreover, topical treatment with Tacrolimus did not involve any substantial toxic effects. The patients most often
complained of local irritations in the area where the ointment was applied, which were transient by nature and did not demand
the drug withdrawal. Today Tacrolimus is a valuable and safer alternative to glucocorticosteroids for treatment of atopic dermatitis.
It can also be used for treating other dermatoses, which demands additional all-round examination.

Intestinal helminth and protozoan infections in patients withchronic dermatoses
Abstract
According to the statistics reports for 2005-2008, a reliable reduction in the intensive indices of lambliasis incidence rate in the
Sverdlovsk region as well as tendency towards a reduction in opisthorchosis and enterobiasis incidence rate was revealed. An
examination of 262 patients in the clinics of FGU UrNIIDViI revealed that the incidence of helminthic and protozoal invasions in
psoriatic patients was 44.1% while the incidence of allergic dermatoses was 32.3%. Blastocysts, intestinal amoebas, seatworms
as well as opisthorchid flukes in case of psoriasis dominated in terms of incidence. High prevalence of seatworms in adolescent
males and women of childbearing potential was discovered.

Combination treatment of psoriasis patients with kartalinointment and ehf therapy
Abstract
The author assessed clinical efficacy of a combination treatment of psoriasis patients with EHF therapy and Kartalin ointment.
Clinical efficacy of combination treatment of psoriasis patients using the Kartalin ointment and EHF therapy was assessed in 153
patients suffering from mild to moderate psoriasis.
All of the subjects were divided into three groups. The first group comprised 30 people receiving conventional treatment (vitamins,
hyposensitization drugs and indifferent ointments) and the second group comprised 50 patients receiving conventional treatment
in a combination with EHF therapy. The third group comprised 73 patients receiving EHF therapy in a combination with Kartalin
ointment and conventional treatment.
It was established that extremely high frequency (EHF) radiation (4.7-5.1 mm) in a combination with Kartalin ointment had the most
evident effect on the clinical and morphological indices in psoriatic patients at the progression stage of the disease accompanied
by the decrease in PASI by 67.2%.

Possibility to use Ademetionine for the treatment of psoriaticpatients with a pathology of the hepatobiliary system
Abstract
The endogenous intoxication syndrome manifested in the form of excessive accumulation of low and mean molar mass substances
in plasma and urine as well as increased blood plasma concentrations of malonic dialdehyde against the background
of reduced alpha-tocopherol was revealed in psoriatic patients with the concomitant hepatobiliary system pathology. Application
of Ademetionine in the dose of 800 mg per day for 15 days as a part of the complex therapy for such patients as opposed to the
patients receiving standard therapy resulted in reliable reduction in the severity of clinical manifestations of the disease and PASI
value as well as decrease in the concentration of low and mean molecular mass substances in plasma and erythrocytes, reduction
in the blood plasma malonic dialdehyde concentration and increase in alpha-tocopherol.

Efficient application of external softening and moisturizingdrugs for the treatment of atopic dermatitis in children
Abstract
The article presents data confirming the efficacy of external application of Locobase Repair cream and Atoderm cream in a combination
with hydrocortisone 17-butyrate 0.1% (Locoid ointment) in children aged from 6 months to 12 years suffering from moderate
atopic dermatitis. The patients were followed up for a year. The treatment efficacy was estimated according to the dynamics of
the SCORAD index and skin instrumental examination indices (corneometry, tewametry and pH-metry). The life quality in children
younger than 5 years was estimated based on the Childrens Dermatology Life Quality Index (CDLQI). The external therapy mentioned
above resulted in the decreased frequency of atopic dermatitis exacerbations, their reduced severity, increased duration of
inter-recurrent periods and reduced need in topical corticosteroids.
