Control materials used for diagnostics of syphilis: types and methods for ensuring stability
Abstract
The authors present data about the types of control materials used for diagnostics of serological syphilis, examine problems related to the development of control materials with or without anti-syphilis pathogen antibodies on the basis of the human blood serum. The authors substantiate the advantages of using liquid native control materials as compared to their lyophilic analogs. The article evaluates the effect of a body of physical and chemical factors on the preservation of the specific activity of protein structures in human blood serum samples stored in the native and lyophilic forms. The possibility to use different stabilizers for long-term preservation of the specific activity of antibodies forming a part of control materials is discussed.

Experience of molecular typing and phylogenetic analysis of N. gonorrhoeae strains in the Russian Federation
Abstract
The article presents results of the molecular typing and phylogenetic analysis of N. Gonorrhoeae strains in the Russian Federation conducted based on a large sample of N. Gonorrhoeae strains. A considerable genetic variety of N. Gonorrhoeae strains circulating in the territory of the Russian Federation was revealed, which can serve as an evidence of a high rate of accumulation of por and tbp gene mutations among the Russian population of N. Gonorrhoeae strains. The authors established the genetic relationship between individual sequence types of N. Gonorrhoeae strains, and revealed total and dominating sequence types of N. Gonorrhoeae strains found both in different territories of the Russian Federation and abroad, which confirms that such strains can be transferred between different territories of the Russian Federation and from abroad due to active migration of population.

Selection of the therapy of herpes viral infection based on the data of the monitoring of the sensitivity of the herpes simplex virus to etiotropic drugs
Abstract
The authors present the results of the monitoring of the sensitivity of herpes simplex virus (HSV) strains in vitro isolated from 110 female patients with various forms of the herpes virus infection to etiotropic drugs (acyclovir (ACV)), valacyclovir) based on the data provided by the phenotypic method. The authors established that 90% of HSV strains were sensitive to ACV irrespective of the isolation conditions, and the share of non-sensitive strains was 10% while the share of strains that were not sensitive to valacyclovir was not higher than 0.9%. These data are of great value for the evaluation of the pathological process activity, forecast for the disease and selection of an individualized treatment regimen for patients providing for the selection of an antiviral drug and its optimum dose in view of the results of virology studies.

Pharmacoeconomic aspects of treatment of psoriasis with biological drugs
Abstract
Psoriasis is an autoimmune skin disease substantially reducing the patients’ quality of life. Practical application of biologic drugs made it possible to achieve substantial progress in the psoriasis therapy. Goal of the article: comparative evaluation of the efficacy of etanercept, infliximab, adalimumab and ustekinumab costs in psoriatic patients. Methods. The cost efficacy was evaluated based on the results of randomized placebo-controlled studies. The analysis was conducted by the cost/efficiency method from the point of view of the health care system. Reduction in the severity of the disease by the PASI index vs. baseline was used as a criterion to evaluate the clinical efficacy. The evaluation was carried out for a course of therapy lasting for 52 weeks; the analysis took into consideration data on all patients (both responding and not responding to the treatment in 12 weeks) as well as patients failing to respond to the treatment in 12 weeks and withdrawing from the therapy. Costs and clinical efficacy were assessed for the time period equal to one year. The costs per one patient achieving PASI 75 were taken as the cost/efficacy coefficient. Results: treatment with adalimumab was shown to be characterized by a lower cost/efficacy coefficient vs. etanercept; infliximab or ustekinumab. Withdrawal in case of non-responding patients in 12 weeks reduces the therapy costs by 17%—40%. In this case, adalimumab proves to be more cost efficient. Conclusions. Treatment of psoriatic patients with adalimumab is expedient from the clinical and economic points of view since it is characterized by the minimum cost/efficacy ratio against the background of high clinical efficacy vs. etanercept; infliximab or ustekinumab.

Role of the Helicobacter pylori as a trigger factor in the development of rosacea and effect of its eradication on the course of dermatosis
Abstract
The authors conducted an analysis of diseases of the gastrointestinal tract, a study of morphology and functional activity of the mucous coat of different stomach sections, determination of H. pylori, determination of the cytokine profile (anti-inflammatory cytokines) in 55 patients with different forms of rosacea vs. 20 healthy volunteers and 55 patients from the risk group consulted or treated by a gastroenterologist because of gastrointestinal disorders but without any manifestations of dermatosis. Moreover, the authors evaluated the effect of the eradication of H. pylori on the course of the disease and cytokine profile of the immune response in patients suffering from rosacea. It was shown that the group of patients with rosacea have much fewer cases of stomach diseases (gastritis, gastroduodenitis, ulcer) than the group of patients with an increased risk of the development of these diseases. On the contrary, patients with rosacea have more cases of H. pylori (83%) than the group of healthy volunteers (20%) and group of patients with stomach diseases (47%) though the bacterial load is lower (I—II vs. III—IV in the risk group). Indices of the cytokine profile demonstrate that patients with rosacea form an immune response based on the mixed Th1/Th2 response type. This results in the microbial persistence, chronic inflammation and possible autoimmune reactions lasting even after the eradication of H. pylori and improvement of the clinical picture of the disease, which possibly explains the chronic course of the disease and development of recurrences.

Angiolymphoid hyperplasia with eosinophilia and generalized affection of the skin
Abstract
Doctors from the State Research Center for Dermatology, Venereology and Cosmetology followed up a female patient with a rare diagnosis: angiolymphoid hyperplasia with eosinophilia. The clinical nature of the skin had a number of specific features: propagation of eruptions all over the skin as well as double-side scrotal lymphadenopathy. The count of eosinophils in the total blood count was normal, which complicated the diagnostics. The authors provide data from literature sources on the etiology and pathogenesis, clinical features, differential diagnostics methods and therapy of the disease.

First clinical experience of the use of ustekinumab (Stelara) for treatment of psoriatic patients in Russia
Abstract
The authors examined the efficacy and safety of treatment of patients suffering from moderate to severe forms of psoriasis with ustekinumab (blocker of р40 subunit of interleukin-12 and interleukin-23) taking into account the PASi index. They revealed high clinical efficacy of ustekinumab for treatment of psoriatic patients.

Role of modern topical steroids in treatment of atopic dermatitis in children
Abstract
Atopic dermatitis is a widespread problem, and its treatment is a complicated problem for clinicians. The efficacy, safety, cost and complexity of therapeutic measures are the Key components for treatment compliance and ultimately define the success of treatment. The article presents the results of a prospective open-label study of the clinical efficacy of the application of mometasone furoate (Elocom) for atopic dermatitis in children with the exudative and erythematous squamous forms with initial signs of lichenification and its effect on the IDLQI (Infant's Dermatological Life Quality Index). It was established that the application of Elocom vs. the hydrocortisone cream and ointment (in case of the exudative form) contributed to the faster reduction of the intensity of clinical manifestations of atopic dermatitis.

On the results of an open-label single-arm study to evaluate the efficacy, safety and tolerability of acnecutan in patients suffering from severe forms of acne as well as acne resistant to other therapies
Abstract
Goal: evaluation of the efficacy and safety of acnecutan in patients with severe forms of acne or if other acne treatment methods prove to be inefficient. Materials and methods: the study involved 35 patients with severe forms of acne and acne resistant to other therapies. The patients received a monotherapy with acnecutan. The efficacy and safety of this therapy were evaluated at each visit judging by the clinical picture of the disease; the severity of the disease was defined based on the count of components (such as comedones, papulopustules, nodes, dyschromias and scars). The safety of the drug was assessed based on the data on adverse and unforeseen events taking into account subjective (patient’s complaints) and objective (total blood count, urine test, blood biochemistry) indices. Results: against the background of the course of treatment with acnecutan, all patients who participated in the study clinical achieved remission. Conclusion: acnecutan is a highly efficient drug for the treatment of severe forms of acne or acne resistant to other treatments.

Efficiency of the therapy of onychomycosis with irunine (based on the results of a multicenter study conducted in 2010 and 2011)
Abstract
The article presents the results of a multi-center study of the clinical efficiency of the use of Irunine ® (itraconazole) for the treatment of 1,340 patients with onychomycosis. Patients were administered two capsules (100 mg) of Irunine ® twice a day after a meal for 7 days (as a pulse therapy) with a three-week break. The duration of treatment was 3—5 months. Clinical recovery was achieved in 74.9% of cases, and stable improvement was observed in 23.4% of patients.

Clinical efficacy, safety and tolerance of 0.1% Tacrolimus ointment in the treatment of atopic dermatitis in the moderate and severe forms
Abstract
Goal: to evaluate the clinical efficacy of 0.1% Protopic ointment in the treatment of atopic dermatitis as a part of a complex therapy. Materials and methods. The study involved a group of 30 patients of both sexes who were diagnosed with atopic dermatitis in the moderate and severe forms at the age of 17—40, who received the 0.1% Tacrolimus ointment (which was applied on the affection foci) as a part of a complex therapy twice a day. The efficacy and safety of this therapy were evaluated based on the SCORAD, Dermatology Life Quality Index (DLQI), indices of personal and situational anxiety, and subjective patient’s feelings. Results. The application of the Protopic ointment resulted in a high total percentage of clinical recovery and significant improvement (86.2%) as well as fast (on Day 7) achievement of a positive effect. Conclusion. The Protopic ointment is characterized by high efficacy and good tolerance when used for the treatment of moderate to severe forms of atopic dermatitis as a part of the complex therapy.

Abstract
The article is considering contemporary views on the role and significance of microorganisms in pathogenesis of atopic dermatitis. One can also find here efficiency analysis of cosmetics Emolium P «Triaktivnyj cream» and «Triaktivnaja bath emulsion» as treatments for low and middle severity atopic dermatitis, as well as research results of skin microbiota (before study initiation and on 14 day).
